Affidavit of Correction DOT
An Affidavit of Correction for DOT testing is a document used to correct errors on the Federal Drug Testing Custody and Control Form (CF). This form is essential for ensuring that the drug test is not cancelled due to errors. The affidavit must be signed by the collector of the drug test and certifies that the errors occurred and that appropriate measures have been taken to prevent their recurrence. The affidavit must be completed by the collection site personnel upon arrival at the site and returned to the employer with the employer's copy of the Chain of Custody Form.

Designated Employer Representative Training (USCG)
